New Module Error- RACHEL +3.0

Hi Team,

I created a new module called ‘Flexible Open and Distance Education -FODE’. I successfully uploaded the module to RACHEL Plus 3.0, however when I tried to open the link, I am receiving the error below. Do anyone has the fix to this problem? Your response is greatly appreciated.
